One of the major things to know about PDFs is that consumers prefer them almost 400% more than filling out a form that requests a printed copy to me mailed to them, according to Marketing Sherpa.
What seems to work best is to have the PDF offer positioned above the mail-a-brochure form for the visitors ease of use. You still need to offer the option of a form as a small, but significant number of visitors will still prefer the printed option.
You hear both sides of the story concerning whether it is more important for you to collect names by having some type of form to fill out before you are allowed to download the PDF. While it depends upon your overall purpose, you need to know that approximately 95% of people will bypass the PDF download altogether if they have to go through the extra step. I wouldn't include it if I were you. You could always offer a teaser of some sort to get them to sign up and give out their information.
Consider the tradeoff between the two: Would you rather have thousands of your PDF files on your visitors computers or devices, or hundreds of registrations. Most the time the thousands of PDF files will be far more important. Consider that in your marketing and web site setup.
